Тест- смужки для визначення рівня холестерину в крові №25 Постанова КМУ №1178 від 12.10.2022 р. п. 10 (із змінами) Постачальник постачає товар на адресу Замовника: 53003, Україна, Дніпропетровська область, Криворізький район, с. Новомайське, вул. Вишнева, … 1А, (2-й поверх) до 29.12.2023 року за Заявкою Замовника на товар. Термін придатності Товару на момент поставки повинен становити не менше 80% від загального терміну придатності. Термін придатності на момент поставки складати не менше 12 місяців. Постачальник повинен поставити Замовнику товар, якість якого відповідає діючому законодавству України, стандартам та правилам, медико-технічним вимогам даного виду товару і підтверджується сертифікатами якості виробника, які надаються при кожній поставці товару. Приймання-передача Товару за кількістю здійснюється відповідно до видаткової накладної та Специфікації. - по кількості відповідно до товаро-супровідних документів на товар, прибуткової накладної із зазначенням торговельної назви, терміну придатності; декларації на відповідність або свідоцтва про державну реєстрацію – для виробів медичного призначення; наявності інструкції про застосування медичного виробу на кожне найменування відповідної накладної; - по якості – відповідно до документів, що засвідчують його якість.
