Князя Романа, 38; м.Львів, вул. Метрологічна, 12; м.Львів, вул. Софії Яблонської,26; м.Дрогобич; м. Червоноград; Волинська обл., м.Луцьк, вул.Світла,4 Термін дії талонів не менше 3 місяців від дати їх отримання
