код ДК 33140000-3 Медичні матеріали (33141310-6 Шприци) (код НК 024:2023 - 47017 Шприц загального призначення одноразового використання)

Поставка товару протягом 2 робочих днів з дати отримання заявки від покупця але не пізніше 25.12.2023р. Доставка товару здійснюється транспортом Учасника, за рахунок Учасника, до складського приміщення Замовника за адресом: 69057, місто Запоріжжя, … вулиця Тамбовська, будинок 6, кімната зберігання. Передача товару повинна здійснюватися виключно представником Учасника уповноваженій особі Замовника, на складі Замовника. Всі розвантажувальні та завантажувальні роботи здійснюються Учасником за власний рахунок. Товар повинен постачатися Замовнику у тарі, яка забезпечує зберігання при транспортуванні та відповідає установленим стандартам. Маркування - згідно діючих ТУ та ДСТУ. Товар повинен передаватися Замовнику представником в упаковці підприємства виробника, яка не повинна бути деформованою або пошкодженою. Під час поставки надати, копії декларації відповідності, в якій зазначено предмет закупівлі — для медичних виробів, які пройшли процедуру оцінки відповідності згідно з Технічним регламентом щодо медичних виробів, затвердженим постановою Кабінету Міністрів України від 2 жовтня 2013 р. № 753, копії інструкцій з застосування, або інших документів, які підтверджують якість запропонованого товару та відповідність технічним характеристикам, що вимагає Замовник. Термін придатності на момент поставки повинен бути не менше ніж 80 % від терміну придатності визначеного виробником.
