Файл у форматі pdf, відсканований з листа, складеного в довільній формі і підписаного уповноваженою особою учасника, з обов’язковим зазначенням назви учасника, коду ЄДРПОУ, реквізитів (адреса, телефон (факс, електронна адреса – у разі наявності)); юридичної адреси, банківських реквізитів обслуговуючого банку, відомостей про керівництво (прізвище, ім'я, по-батькові, посада, контактний телефон) із зазначенням дати складання листа. не можливо проаналізувати (не відкривається файл в системі «Prozorro»). 3. Якщо учасник є акціонерним товариством - файл у форматі pdf, відсканований з офіційного витягу з реєстру акціонерів публічних та приватних акціонерних товариств, із зазначенням переліку осіб, які володіють більш ніж 25% статутного фонду, та вказанням, якою частиною статутного фонду вони володіють не можливо проаналізувати (не відкривається файл в системі «Prozorro»).
