Заява на участь в аукціоні подається в електронному вигляді шляхом заповнення електронних форм з окремими полями та завантаження необхідних документів і повинна містити інформацію про закриту цінову пропозицію. Закрита цінова пропозиція не може бути нижча стартової ціни лота. До заяви на участь в аукціоні додаються такі документи: 1. Для фізичних осіб — підприємців та юридичних осіб резидентів — витяг з Єдиного державного реєстру юридичних осіб, фізичних осіб — підприємців та громадських формувань України, (ФОП додає копію паспорта та довідки РНОКПП); для юридичних осіб нерезидентів — документ про реєстрацію у державі її місцезнаходження (витяг з торговельного, банківського або судового реєстру тощо), засвідчений згідно із законодавством держави його видачі, перекладений на українську мову; 2. Заяву в довільній формі про непоширення на заявника — ФОП чи юридичну особу або її посадових осіб (керівника, його заступників та осіб, що їх заміщують) чи її засновників (юридичних та фізичних осіб) спеціальних економічних та інших обмежувальних заходів (санкцій) згідно із Законом України “Про санкції” та актами, прийнятими відповідно до зазначеного Закону (зразок цієї заяви додається в окремому файлі); 3. Довідку довільної форми, що містить наступну інформацію: - Повна назва учасника; - контактний номер телефону; - реквізити рахунка заявника, на який повинен бути повернутий гарантійний внесок; - адреси електронної пошти для надіслання повідомлень системи електронних торгів оператора та організатора аукціону; юридична особа додатково зазначає у довідці: код згідно з ЄДРПОУ; прізвище, ім’я та по батькові представника юридичної особи, серію та номер документа, що посвідчує його особу; реквізити документа, що підтверджує повноваження представника юридичної особи (статут, наказ або протокол/рішення про призначення);
