1. Майно може бути використане потенційним орендарем виключно за цільовим призначення об’єкта оренди. 2.Потенційний орендар повинен забезпечити проведення господарської діяльності з медичної практики. 3.Зобов’язаний надати документ, що підтверджує наявність ліцензії на провадження господарської діяльності з медичної практики. 4.У разі визначення необхідності у ремонтних роботах в орендованому приміщенні здійснити їх за рахунок власних коштів Орендаря та за погодженням Орендодавця. 5.Вимоги до приміщення та його використання - використовувати приміщення строго за призначенням – кардіологічна, ортопедотравматологічна реабілітація та бальнеологія. 6.Вимоги до обладнання: забезпечити наявність якісного сучасного фізіотерапевтичного (апарат ударно-хвильової терапії, високочастотної магнітотерапії, високоінтенсивної лазеротерапії, міостимуляції) та реабілітаційного обладнання в т.ч. апарат для пасивної механотерапії суглобів, обладнання для надання «підвісної терапії» (нейром’язової активації). Учасник надає … підтвердження відповідності даного параметру. 7.Проведення інструментальної діагностики для кардіологічних та ортопедо-травматологічних пацієнтів: - кардіологічних пацієнтів - Холтер ЕКГ, Холтер ДМАТ, кардіопульмональне тестування; - травматологічних пацієнтів – лабораторія ходьби; - вертебро-неврологічних та ревматологічних – нейром’язове тестування. 8.Проведення бальнеологічних процедур неврологічним, травматологічним і ревматологічним пацієнтам (наявність апарату душ Шарко, підводний душ масаж, ванни з екстрактами, душ Віші. 9.Укласти договір відшкодування комунальних та інших послуг.
