Additional conditions for the lease of the property:
1. Орендар зобов’язаний працевлаштувати одного працівника. 2. Орендар зобов’язаний мати впроваджену і задокументовану систему управління якістю, з метою недопущення до кінцевого споживача, невідповідної продукції. 3. Орендар повинен підтримувати функціонал електронного рецепта та укласти договір реімбусації з національною службою України, який має включати відпуск лікарських засобів за групами станів (нозологій): серцево –судинні та цереброваскулярні захворювання, цукровий діабет 2 типу та хронічні хвороби нижніх дихальних шляхів, нецукровий діабет, інсуліни людські (флакони безоплатно), інсуліни аналоги (безоплатно), інсуліни аналоги (з доплатою), інсуліни людські (катриджі та шприцручки з доплатою), інсуліни (людські катриджі та шприцручки безоплатно), розлади психіки та поведінки, епілепсія. 4. Орендар повинен бути учасником державної програми «Доступні ліки». 5. Орендар повинен здійснювати безоплатний або пільговий відпуск лікарських засобів за рецептами лікарів згідно Постанови Кабінету Міністрів … України від 17 серпня 1998 р. № 1303 «Про впорядкування безоплатного та пільгового відпуску лікарських засобів за рецептами лікарів у разі амбулаторного лікування окремих груп населення та за певними категоріями захворювань». 6. Орендар повинен здійснювати пільговий відпуск препаратів інсуліну згідно «Порядку відшкодування вартості препаратів інсуліну» затвердженого Постановою Кабінету Міністрів України від 23.03.2016 р. №239. 7. Орендар (крім фізичних осіб – підприємців) зобов’язаний сплачувати ПДФО за найманих працівників за місцем здійснення діяльності (місцевого бюджету Деражнянської міської територіальної громади).
