Note:
1. У зв’язку зі звільненням Застрахованої особи – Бонцой Віктора Степановича, 10.11.1952р.н. та прийняттям на вакантну посаду водія – Андрєєва Степана Петровича, 13.09.1961р.н, Сторони вирішили виключити з Додатку №1 до договору від 03.10.2018 №ОНВ-Ю/03101804/125-В-ІЗФ-18 працівника під №1, а на дане вільне місце внести інформацію щодо Застрахованої особи Андреєва С.П.
2. Ця Додаткова угода набирає чинності з моменту її підписання Сторонами та скріплення печатками і діє протягом строку дії Договору.
3. У всьому іншому, що не врегульовано цією Додатковою угодою, Сторони керуються Договором та чинним законодавством України.
4. Всі інші умови Договору, не змінені цією Додатковою угодою, залишаються чинними у редакції, викладеній в Договорі.
5. Ця Додаткова угода є невід'ємною частиною Договору.
6. Цю Додаткову угоду складено українською мовою у двох автентичних примірниках, що мають однакову юридичну силу, по одному примірнику для кожної Сторони
