Внесення змін до специфікації (Додаток №1 до Договору № 67 (Т/Л).
На засідання тендерного комітету було повідомлено, що за результатами проведення процедури відкритих торгів стосовно закупівлі фармацевтичної продукції (код ДК 021:2015 – 33600000-6)(Diazepam (Діазепам), Ketamine (Кетамін), Morphine (Морфін), Fentanyl (Фентаніл)) (ідентифікатор закупівлі - UA-2018-02-12-001977-b) було укладено договір поставки № 67 (Т/Л) від 02.02.2018 р. Але станом на 18.05.2018 р. змінилась форма випуску морфіну, а саме замість морфіну , розчину для ін'єкцій, 10 мг/мл по 1 мл. в ампулі №10 випускається морфін , розчин для ін'єкцій, 10 мг/мл по 1 мл. в ампулі №5, що викликає необхідність внести зміни в договір та оформити зміни додатковою угодою (разом з тим загальна кількість необхідних до закупівлі ампул морфіну та сума договору залишається без змін).
Таким чином на засіданні комітету було запропоновано внести зміни до договору № 67 (Т/Л) від 02.03.2018 р.., а саме зазначити в специфікації дійсну станом на 18.05.2018 р.форму випуску морфіну , а саме морфін , розчин для ін'єкцій, 10 мг/мл по 1 мл. в ампулі № 5 .
