Преамбулу договору: "Замовник: КОМУНАЛЬНЕ НЕКОМЕРЦІЙНЕ ПІДПРИЄМСТВО "МІСЬКА ПОЛІКЛІНІКА №25" ХАРКІВСЬКОЇ МІСЬКОЇ РАДИ, в особі ГОЛОВНОГО ЛІКАРЯ ЗЛИВКИ ЛАРИСИ ВАСИЛІВНИ, що діє на підставі СТАТУТУ, надалі Споживач, з однієї Сторони і
