5. Дана додаткова угода складено в двох примірниках, по одному для кожної із сторін, що мають однакову юридичну силу.
6. Додаткова угода складає невід’ємну частину Договору.

Note:
1.Відповідно до Рішення Дніпропетровської обласної ради від 25.10.2019 року № 516-18/VII Комунальний заклад «Криворізька центральна районна лікарня» Дніпропетровської обласної ради» реорганізовується шляхом перетворення в Комунальне підприємство «Криворізька центральна районна лікарня» Дніпропетровської обласної ради».
2. Зобов’язання КЗ «Криворізька центральна районна лікарня» ДОР» припиняються та переходять до КП «Криворізька центральна районна лікарня» ДОР», яке згідно Статуту є правонаступником.
3.Загальна сума Договору не змінюється. Залишок зобов’язань по Договору на момент укладення цієї Додаткової угоди складає: 21634.80 грн в т.ч. ПДВ
4. Решта умов Договору, зміну яких не передбачено цією Угодою, залишається незмінним і обов’язковим до виконання Сторонами.
5. Дана Угода набирає чинності з моменту укладення та діє протягом строку дії Договору.
6. Дана Угода складена у двох автентичних примірках, по одному для кожної із Сторін, і є невід’ємною частиною Договору.

та з повним виконанням договірних зобов’язань сторони прийшли суму договору на 13,20 грн., в т.ч. ПДВ
Суму договору зменшити на 11101,20 грн
Загальна сума договору складає: 88738,80,00 грн, в т.ч. ПДВ
(Вісімдесят вісім тисяч сімсот тридцять вісім грн. 80 коп)
2. Додаткова угода діє з моменту підписання і діє до «_31__» грудня 2019 року.
3. Всі інші пункти Договору № 12- 30 від 28.01.2019 року залишаються без змін.
