сторони домовились о внесенні змін до абзацу пункту 2.2. Договору від 21.12.2018 №8/12 та викласти його в наступній редакції: “На 2021 рік сума фінансування складає 27293305,98грн. (двадцять сім мільйонів двісті дев'яносто три тисячі триста п'ять грн. 98 коп) у тому числі ПДВ 4548884,33грн. (чотири мільйони п'ятсот сорок вісім тисяч вісімсот вісімдесят чотири грн. 33 коп.), з яких обладнання 24745092,40грн. (двадцять чотири мільйони сімсот сорок п'ять тисяч дев'яносто дві грн. 40 коп.), у тому числі ПДВ 4124182,07грн. (чотири мільйони сто двадцять чотири тисяч сто вісімдесят два грн. 07 коп.) у 2021 році, з яких за рахунок державного бюджету 24726403,98грн. (двадцять чотири мільйони сімсот двадцять шість тисяч чотириста три грн. 98 коп.), у тому числі ПДВ 4121067,33грн. (чотири мільйони сто двадцять одна тисяча шістдесять сім грн. 33 коп.), з яких, обладнання 24726403,98грн. (двадцять чотири мільйони сімсот двадцять шість тисяч чотириста три грн. 98 коп.), у тому числі ПДВ 4121067,33грн. (чотири мільйони сто двадцять одна тисяча шістдесять сім грн. 33 коп.), за рахунок обласного бюджету 2566902,00 грн. (два мільйони п'ятсот шістдесят шість тисяч дев'ятсот дві грн. 00 коп.), у тому числі ПДВ 427817,00грн. (чотириста двадцять сім тисяч вісімсот сімнадцять грн. 00 коп.), з яких, обладнання 18648,42 грн. (вісімнадцять тисяч шістсот сорок вісім грн. 42 коп.), у тому числі ПДВ 3108,07грн. (три тисячі сто вісім грн. 07 коп.).
На 2022 рік сума фінансування складає 4 000 000,00 грн. (чотири мільйони грн. 00 коп) у тому числі ПДВ 666 666,67 грн. (шістсот шістдесят шість тисяч шістсот шістдесят шість грн. 67 коп.), з яких обладнання 00,00грн. (нуль грн. 00 коп.), у 2022 році, з яких за рахунок інших джерел (власні кошти) 4 000 000,00 грн. (чотири мільйони грн. 00 коп) у тому числі ПДВ 666 666,67 грн. (шістсот шістдесят шість тисяч шістсот шістдесят шість грн. 67 коп.), з яких обладнання 00,00грн. (нуль грн. 00 коп.).
На підтвердження виконаних робіт Сторони укладають Акти виконаних робіт (форма КБ-2В) та КБ-3 «Довідка про вартість виконаних будівельних робіт та витрати», які повинні бути підписані обома Сторонами.”.
Також внести зміни у додаток №3 «План фінансування».
Сторони погодили внести зміни у п.15.1 договору від 21.12.2018 року №8/12 виклавши його в наступній редакції: “15.1. Цей Договір набуває чинності з дати його підписання і діє до 31.12.2022р. з урахуванням вимог Закону України “Про публічні закупівлі”, а в частині виконання зобов'язань по цьому договору, до їх повного виконання Сторонами”
