Note:
1. Сторони домовились продовжити дію Договору на строк до 31.01.2021 року достатній для проведення процедури закупівлі на початку наступного року в обсязі, що не перевищує 20% суми, визначеної у Договорі.
2. Сторони дійшли згоди про внесення змін до умов Договору, а саме пункт 5.1. та пункт 8.1 Договору викласти в наступній редакції:
«5.1. Строк надання послуг – до 31 грудня 2021 року та з 01 січня 2022 року до 31 січня 2022 року.»
«8.1. Цей Договір набирає чинності з моменту його підписання і скріплення його печатками Сторін і діє до 31.01.2022 року, а в частині сплати – до повного виконання фінансових зобов’язань».

1. Сторони домовились продовжити дію Договору на строк достатній для проведення процедури закупівлі на початку 2022 року в обсязі, що не перевищує 20% суми, визначеної у Договорі, у зв'язку з чим збільшується вартість послуги на 37 180,00 грн. (тридцять сім тисяч сто вісімдесят грн. 00 коп.), відповідно пункт 2.1. Договору викласти в новій редакції:
"2.1. Ціна цього Договору складає 223 098,84 грн. (двісті двадцять три тисячі дев'яносто вісім грн. 84 коп.), в т.ч. ПДВ -37 183,14 грн., в тому числі на 2021 рік - 185 918,84 грн. (сто вісімдесят п'ять тисяч дев'ятсот вісімнадцять грн. 84 коп.), в т.ч. ПДВ - 30 986,47 грн. та на 2022 рік - 37 180,00 грн. (тридцять сім тисяч сто вісімдесят грн. 00 коп.), в т.ч. ПДВ -
6 196,67 грн. і включає в себе усі податки і збори та всі інші витрати, пов’язані з наданням послуг, в т. ч. вартість витратних матеріалів, що надаються Виконавцем протягом усього строку дії Договору, інше, згідно умов цього Договору."
