Note:
1. Дію Договору № 752/226-33/24-84 від 12 жовтня 2016 р. продовжити до
23 січня 2017 р. включно. У зв’язку з цим внести зміни у пункти 5.2 і 10.2, виклавши їх у новій редакції:
«5.2. Строк надання Послуг: з 01.09.2016 р. (відповідно до частини третьої статті 631 Цивільного кодексу України) до 23.01.2017 р.»;
«10.2. Цей Договір набирає чинності з дати його підписання і діє до 23.01.2017 р., але в будь-якому разі до повного виконання Сторонами своїх зобов’язань за Договором.

1. Виконавець зобов’язується надати Замовнику послуги в порядку та на умовах, визначених в Договорі № 752/226-33/24-84 від 12 жовтня 2016 р., з урахуванням змін, внесених до нього Додатковим договором № 1/24-149 від 30 грудня 2016 року.
2. Загальна вартість послуг, зазначених у пункті 1 цього Додаткового договору, становить 5 000,00 грн. (п’ять тисяч гривень 00 коп.) без ПДВ , що не перевищує 20 % суми, визначеної в Договорі № 752/226-33/24-84 від 12 жовтня 2016 р.
