1. В зв’язку з перебуванням директора Комунального некомерційного підприємства Охтирської міської ради "Охтирська центральна районна лікарня" Збаражського Петра Петровича, станом на дату укладення Договору 02.08.2021 року, на лікарняному по листку непрацездатності, Сторони дійшли згоди внести зміни до Договору, а саме, в преамбулі, в тексті Договору та в Додатках №1, №2, №3, №4 в усіх відмінках змінити назву «Директор Збаражський Петро Петрович» на «Виконуючу обов’язки директора Хоруженко Віту Вікторівну». 2. В додатку № 2 до Договору Комерційна пропозиція для споживачів, які не відносяться до сегменту ринку, обов’язки постачання якому покладені на Постачальника універсальної послуги, розділ "Вартість договору" викласти в новій редакції: "Загальна вартість цього Договору становить 517 550 грн. 00 коп., в тому числі ПДВ 86 258 грн. 33 коп. і є орієнтовною. У тому числі: За рахунок бюджетних призначень 517 550 грн. 00 коп., в тому числі ПДВ 86 258 грн. 33 коп. За рахунок відшкодованих коштів 0 грн., враховуючи ПДВ 0 грн."
