В абзаці першому Договору посаду "головний лікар" ДЗ "УСДРЗН МОЗ України" змінити на посаду директора ДЗ "УСДРЗН МОЗ України"у зв’язку з введенням в дію з 01.09.2019р. нового штатного розпису, згідно наказів МОЗ України від 15.08.2019 № 1799 та від 31.10.2018 № 1977, в якому змінена назва посади керівника закладу з "головний лікар" на "директор". Всі інші умови договору залишаються без змін.
