Note:
1. Згідно з п.10.3 договору сторони дійшли згоди продовжити термін дії договору до 31.03.2020 на суму 184 426,00 грн. (сто вісімдесят чотири тисячі чотириста двадцять шість грн. 00 коп.), у тому числі 30 737,67 грн. ПДВ, що не перевищує 20 відсотків суми 923 525,00 грн., з ПДВ, визначеної у договорі, якщо видатки на цю мету затверджено в установленому порядку у відповідності до вимог ч.5 статті 36 Закону України «Про публічні закупівлі».
2. У відповідності до пункту 5 ч. 4 статті 36 Закону України «Про публічні закупівлі» зменшити вартість 1л бензину А-92 на 2,6 грн., з ПДВ, встановивши її у розмірі 23,90 грн., у тому числі 3,98 грн. ПДВ, за 1 л.
3. У відповідності до пункту 5 ч. 4 статті 36 Закону України «Про публічні закупівлі» зменшити вартість 1л. дизельного пального на 1,6 грн., з ПДВ, встановивши її у розмірі 24,90 грн., у тому числі 4,15 грн. ПДВ, за 1 л.
14. Сума договору після внесення змін. 184 426,00 грн.
15. Кількість товару або обсяг виконання робіт чи надання послуг після внесення змін. 7 540 л.
