Note:
1. ЗАМОВНИК та УЧАСНИК дійшли взаємної згоди внести зміни до Договору виклавши пункт 1.2. розділу І. "ПРЕДМЕТ ДОГОВОРУ" в новій редакції:
"1.2. Найменування (номенклатура, асортимент) товару:
Код ДК 021:2015 (СРV): 15510000-6 - Молоко та вершки
Сметана – 20 % жир. – 702,93 кг.
Молоко – 2,5% жир. – 391,17 кг.
Усього: 1094,1 кг.
2. Внести зміни до Договору виклавши пункт 3.1. розділу ІІІ. "ЦІНА ДОГОВОРУ" в новій редакції:
"3.1. Ціна цього Договору становить 94 882,00 гривень (Дев'яносто чотири тисячі вісімсот вісімдесят дві гривні 00 копійок) у т.ч. ПДВ 15 813,67 грн."
3. Внести зміни до специфікації до Договору, виклавши її в новій редакції, що додається.
4. Інші умови вищевказаного Договору залишаються без змін.
5. Дана Додаткова угода набирає чинності з "16.02.2022 року, і є невід’ємною частиною Договору про закупівлю товарів від 01 лютого 2022 року № 06
6. Невід'ємною частиною цієї Додаткової угоди є специфікація.
7. Цю Додаткову угоду складено у двох оригінальних примірниках, що мають однакову юридичну силу, по одному примірнику для кожної із Сторін.
