Note:
1. Керуючись п.1, ч.5, ст.41 Закону України "Про публічні закупівлі", у зв'язку із відключенням точок до послуги Інтернет, які обслуговуються АТ «Укртелеком», у відповідності до листа ОКНП "Чернівецька обласна клінічна лікарня" від «22» вересня 2021р. № 2991 Сторони, згідно п. 2.2 та п. 5.5.1 Договору № 1228 від 01 липня 2021 р., дійшли згоди:
1.1. Внести зміни в перелік точок підключення до послуги Інтернет, шляхом відключення точок з «___» вересня 2021 року згідно Додатку 2 до Угоди від 01 липня 2021р. №1228/2.
1.2. У зв'язку із відключенням кількості точок зменшити ціну Договору на суму 21 888,00 грн.(двадцять одна тисяча вісімсот вісімдесят вісім грн. 00 коп.)та викласти пункт 2.1. Договору в наступній редакції:
«Ціна цього Договору становить 100 755.52 грн. (сто тисяч сімсот п'ятдесят п'ять грн.52 коп.)
2. Усі інші умови Договору № 1228 від 01 липня 2021 року залишаються без змін.
