Note:
1. Керуючись п. 8 ч. 5, ч. 6 ст. 41 Закону України «Про публічні закупівлі», пунктом 11.1. Договору Сторони домовились продовжити строк дії Договору для проведення процедури закупівлі на початку наступного року, що не перевищує 20 відсотків суми, визначеної в початковому договорі про закупівлю, укладеному в попередньому році.
2. Сторони прийшли до домовленості внести зміни до Договору, пункти 3.1. та 10.1., читати в наступній редакції:
«3.1. Ціна Договору становить 4667201,60 (чотири мільйони шістсот шістдесят сім тисяч двісті одна гривня 60 копійок) гривень, у тому числі податок на додану вартість 777866,93 (сімсот сімдесят сім тисяч вісімсот шістдесят шість гривень 93 копійки), ціна договору на 2021 рік становить 3897201,60 (три мільйони вісімсот дев’яносто сім тисяч двісті одна гривня 60 копійок) гривень, у тому числі податок на додану вартість 649533,60 (шістсот сорок дев’ять тисяч п’ятсот тридцять три гривні 60 копійок). Ціна цього договору на січень-березень 2022 року становить 770000,00 (сімсот сімдесят тисяч гривень 00 копійок) у тому числі ПДВ 128333,33 (сто двадцять вісім тисяч триста тридцять три гривні 33 копійки).»
«10.1. Договір набирає чинності з моменту його підписання і діє до 31.03.2022 року».
