Внесення змін у відповідності до ст. 36 Закону "Про публічні закупівлі", а саме: п. 3 ч. 4 покращення якості предмета закупівлі за умови, що таке покращення не призведе до збільшення суми, визначеної в договорі;
Підстава: Рішення обласної ради від 22.02.2019 № 941-VII "Про внесення змін до рішення обласної ради від 21 грудня 2018 року № 913-VII "Про обласний бюджет Одеської області на 2019 рік" та ст. 23 Бюджетного кодексу України. Постанова КМУ № 983 "Порядок надання субвенції з державного бюджету місцевим бюджетам на здійснення заходів, спрямованих на розвиток системи охорони здоров'я у сільській місцевості".
Інформація про зміни:
- Фінансування на 2019 рік у сумі 8 622 131,03 грн., з них 10% cпівфінансувавння у сумі 862 213,10 гривень з рахунку № 35426149005967, МФО 828011, ГУ ДКСУ та 90% фінансування за рахунок субвенції у сумі 7 759 917,93 гривень з рахунку № 35425149705967 МФО 828011, ГУ ДКСУ;
- Внести зміни до розділу VI "Порядок здійснення оплати", а саме п. 6.1 доповнити таким змістом: "Замовник" перераховує "Підряднику" аванс на придбання матеріалів у розмірі 30% від вартості договору, а саме - 2 586 639,31 грн. (Два мільйони п'ятсот вісімдесят шість тисяч шістсот тридцять дев'ять гривень 31 копійка), у тому числі ПДВ (20%) - 517 327,86 грн. (П'ятсот сімнадцять тисяч триста двадцять сім гривень 86 копійок) по об'єкту: "Амбулаторія загальної практики сімейної медицини по пров. Новоселів, 3а, в с. Кам'янка Роздільнянського району - будівництво"...
- Усі інші умови договору залишаються без змін.

Note:
Сторони уклали цю додаткову угоду про наступне:
1. Підстава: Рішення обласної ради від 22.02.2019 №941-VII «Про внесення змін до рішення обласної ради від 21 грудня 2018 року №913-VII «Про обласний бюджет Одеської області на 2019 рік» та ст.23 Бюджетного кодексу України, ЗУ «Про публічні закупівлі».Постанова КМУ №983 «Порядок надання субвенції з державного бюджету місцевим бюджетам на здійснення заходів, спрямованих на розвиток системи охорони здоров’я у сільській місцевості».
2. Фінансування на 2019 рік у сумі 8 622 131,03 грн. з них:10% співфінансування у сумі – 862 213,10 гривень з рахунку №35426149005967, МФО 828011, ГУ ДКСУ та 90% фінансування за рахунок субвенції у сумі -7 759 917,93 гривень з рахунку №35425149705967 МФО 828011 ГУ ДКСУ.
3. Внести зміни до п.3.1 розділу III «Ціна договору» та викласти в наступній редакції: «Ціна договору становить 8 622 131,03 (Вісім мільйонів шістсот двадцять дві тисячі гривень сто тридцять одна гривня 03 копійок) гривень. З них фінансування за рахунок субвенції державного бюджету у сумі -7 759 917,93 гривень з них по КЕКВ 3122 передбачено фінансування на будівництво та вмонтоване обладнання у сумі – 7 485 265,72 гривень та по КЕКВ 3110 передбачено фінансування у сумі - 274 652,21 гривень на невмонтоване устаткування та меблі, також передбачено співфінансування відповідно умовам КМУ №983 у сумі – 845 768,61 гривень. 2.Внести зміни до розділу X договору, та продовжити термін дії договору до 31 грудня 2019 року, а саме викласти п.10.1 в наступній редакції: «Цей Договір набирає чинності з моменту його підписання і діє до 31.12.2019 року та до повного виконання Сторонами взятих на себе зобов’язань»
4. Усі інші умови договору №6 від 26 лютого 2019 року, відповідно п. 4 ст. 36 Закону України «Про публічні закупівлі», залишаються без змін.
5. Додаткова угода складена у двох примірниках та набирає чинності з моменту підписання сторонами.
Додатки до даної Додаткової угоди є невід’ємною частиною:
1) Додаток №1 – «Календарний графік виконання робіт»;
2) Додаток №2 – «План фінансування робіт»
