У зв’язку з реорганізацією шляхом приєднання з «01»_04__2020 року Комунальне підприємство «Кременчуцький обласний протитуберкульозний диспансер Полтавської обласної ради» приєднано до КОМУНАЛЬНОГО ПІДПРИЄМСТВА «ПОЛТАВСЬКИЙ ОБЛАСНИЙ КЛІНІЧНИЙ ПРОТИТУБЕРКУЛЬОЗНИЙ ДИСПАНСЕР ПОЛТАВСЬКОЇ ОБЛАСНОЇ РАДИ», при цьому КОМУНАЛЬНЕ ПІДПРИЄМСТВО «ПОЛТАВСЬКИЙ ОБЛАСНИЙ КЛІНІЧНИЙ ПРОТИТУБЕРКУЛЬОЗНИЙ ДИСПАНСЕР ПОЛТАВСЬКОЇ ОБЛАСНОЇ РАДИ» є правонаступником Комунального підприємства «Кременчуцький обласний протитуберкульозний диспансер Полтавської обласної ради» та приймає на себе всі його фінансові зобов’язання за даним Договором.
Сторони погодили внести зміни в назву Замовника, а саме Комунальне підприємство «Кременчуцький обласний протитуберкульозний диспансер Полтавської обласної ради» диспансер змінити на КОМУНАЛЬНЕ ПІДПРИЄМСТВО «ПОЛТАВСЬКИЙ ОБЛАСНИЙ КЛІНІЧНИЙ ПРОТИТУБЕРКУЛЬОЗНИЙ ДИСПАНСЕР ПОЛТАВСЬКОЇ ОБЛАСНОЇ РАДИ» та відобразити відповідні зміни в преамбулі Договору та Реквізитах Сторін:

«Договір вважається пролонгованим до 28.02.2021 року на час достатній для проведення процедури закупівлі на 2021 рік, в обсязі, що не перевищує 20% загальної суми, визначеної у договорі, укладеному у попередньому році», Ця Додаткова угода вважається укладеною і набирає чинності з моменту підписання Сторонами та скріплення її печатками Сторін та діє на протязі дії даного Договору.

«Договір вважається пролонгованим до 28.02.2021 року, на час достатній для проведення процедури закупівлі на 2021 рік, в обсязі, що не перевищує 20% загальної суми, визначеної у договорі, укладеному у попередньому році».
«Сума договору складає 112 953,15 грн. (ссто дванадцять тисяч дев'ятсот п'ятдесят три грн. 15 коп.) без урахування ПДВ.
