Note:
1. Внести зміни до договору (рахунку) № АЮ-3701 від 29.04.2026, укладеного з ТОВ «АКЛАС.ЮА», а саме:
- вважати чинним для проведення розрахунків рахунок № АЮ-3714 від 04.05.2026 на суму 20526,17 грн з ПДВ;
- продовжити строк виконання зобов’язань щодо оплати до 07.05.2026;
- продовжити строк постачання товару до 31.12.2026.
2. Підтвердити, що інші умови договору залишаються без змін, зокрема:
- предмет закупівлі: стіл письмовий керівника (ДК 021:2015 – 39121000-6);
- ціна договору: 20526,17 грн з ПДВ (без змін).
3. Підставою для внесення змін є:
продовження строку дії договору про закупівлю та строку виконання зобов’язань у зв’язку із затримкою фінансування витрат Замовника, за умови що такі зміни не призводять до збільшення суми договору.
