Note:
1. На підставі п.5 ч.4 ст. 36 Закону України «Про публічні закупівлі», п.13.4 Договору про закупівлю послуги № 5 від 10.02.2020, Сторони, у зв`язку із доведеною сумою кошторисних призначень та за взаємною згодою, вирішили зменшити ціну Договору та вартість одного діто - дня у розрізі вікових категорій та викласти п.3.1. Розділу ІІІ Договору, Додатки 1, 2 до Договору у новій редакції:
- «3.1. Ціна Договору становить 29240220,35 (двадцять дев’ять мільйонів двісті сорок тисяч двісті двадцять гривень 35 копійок) гривень без ПДВ.
Загальна кількість діто - днів – 1083778.
Вартість одного діто - дня у розрізі вікових категорій становить для дітей:
- віком від 6 до 10 років – 24,50 грн. (400635 діто - днів);
- віком від 10 до 14 років – 27,65 грн. (431613 діто - днів);
- віком від 14 до 18 років – 29,78 грн. (251530 діто - днів).
2. Всі інші умови Договору залишаються без змін і є обов’язковими до виконання Сторонами.
3. Дану Угоду складено українською мовою у двох примірниках, які мають однакову юридичну силу, по одному примірнику для кожної із Сторін та яка вступає в силу з моменту її підписання.
4. Ця Додаткова угода є невід’ємною частиною Договору № 5 від 10.02.2020 р.
