1. Сторони дійшли згоди внести зміни в специфікацію до Договору №21 від 29.05.2018 року та викласти її в новій редакції (Додаток № 1 до Додаткової угоди до Договору) на підставі пп. 3 п. 10.3. Договору та згідно п. 3 ч. 4. статті 36 Закону України «Про публічні закупівлі» (покращення якості предмета закупівлі за умови, що таке покращення не призведе до збільшення суми, визначеної в договорі):
- позиція №4 ДИАЗЕПЕКС® розчин для ін`єкцій, 5 мг/мл по 2 мл в ампулі №10 замінити на аналог СИБАЗОН розчин для ін'єкцій, 5 мг/мл по 2 мл в ампулі №10.

Note:
Відповідно п. 1 ч. 4 ст. 36 ЗУ «Про публічні закупівлі», сторони вирішили зменшити обсяг закупівлі, зокрема з урахуванням фактичного обсягу видатків замовника та внести наступні зміни до Договору:
1. Зменшити загальну суму договору на 46 823,20 грн. (сорок шість тисяч вісімсот двадцять три грн. 20 коп.), у тому числі ПДВ та викласти пункт 4.1. в наступній редакції:
«Сума даного Договору становить: 791 829,96 грн. (сімсот дев’яносто одна тисяча вісімсот двадцять дев’ять грн. 96 коп.) з ПДВ, в т.ч. ПДВ 51 801,96 грн.»
